Diabetic retinopathy is a diabetic microvascular complication caused by eye diseases. Specifically, 80% of diabetic patients with diabetes for 10 years or more have developed diabetic retinopathy; however, preventive eye examinations and treatments can reduce the number of new cases by 90%. If it is possible to diagnose the patient in the early stages of diabetic retinopathy, classified as non-proliferative diabetic neuropathy (NPDR), in which patients with mild or no ophthalmological symptoms are diagnosed, then the number of new cases may be further reduced.

For some diseases, biomarker screening methods can improve the timeliness of detection, and therefore get timely treatment. In the biomarker screening process, the concentration of proteins, metabolites, or other molecules is screened from the patient’s blood or other body fluid samples, and the concentration of these substances is compared with healthy individuals. If the concentration of the molecular species of interest exceeds the normal range, medical professionals can make an accurate diagnosis. If more than one biomarker is monitored, the success rate of diagnosis will also increase.

In a proteomic research paper published by Kim et al. in the journal, they explored the use of a mass spectrometry method called multiple reaction monitoring (MRM) to quantify protein biomarkers for diagnosing the risk of NPDR in patients. They collected plasma samples from non-NPDR patients and patients with mild, moderate, and severe NPDR stages. After denaturation and proteolytic fragmentation of plasma proteins, a standard solution of exogenous peptides is added as an internal standard for quantification. The peptide was separated and analyzed by LC-MS. Using this technique, the authors discovered 28 different proteins at different concentrations in the plasma of non-NPDR patients, mild NPDR patients, and severe NPDR patients. After analyzing the proteins of these 28 potential biomarkers, Kim et al. developed two panels with 4 biomarkers, showing the extremes among patients with mild NPDR, patients with mild + moderate NPDR, and patients with non-NPDR. Great disagreement. The experiment in this paper requires the normalization of the mass spectrometry signal in the plasma sample relative to an exogenously added standard peptide. Because plasma needs to be processed before adding standard peptides, the non-specific adsorption of the plastic surface can lead to the loss of marker proteins. Because this may affect the standard ratio of exogenous addition of biomarker proteins, the results of the generalization relied on the use of low-retention plastic products to reduce protein adsorption.
